一、带以下资料到检测中心申请备案:
1、化工品品备案申请表----原件一份，可在网上下载；
2、申请单位营业执照复印件一份；
3、生产厂家对该化工品品的有毒有害声明；
4、化工品品成份表；
5、标签原件，如标签是外文的，还要译成中文文本；
6、化工品品外观图片 以上资料一式二份，用透明的文件夹夹好，厂家有害声明要盖厂家的公章，其它资料可盖申请单位的公章。
二、以上资料审核合格后，5个工作日后出受理通知书；
三、送样作专项检测；要预先交费，等检测结果出来后，如不够的再补交。
四、15个工作日出测检结果，如合格把检测结果交当初接收你备案的经办人员，3个工作日后[出化工品品备案书]。
