1）进口旧机电产品备案申请书

2）申请人、收货人、发货人营业执照（复印件）

3）装运前预检验申请书

4）拟进口旧机电产品清单（按标准格式填写，一式两份，要求以电脑打印。）

5）进口旧机电产品描述（按网上标准格式填写）

6）其它材料（8年前制造的旧机电产品原则上均需提供产品彩色图片/照片；制造年限久的设备如在发运前已经维修整理，申请人可提供相关记录与证明；进口的二手大型成套设备需提供设备配置图、工艺流程图等资料。）
